THE FOLLOWING ITEMS MUST BE ON FILE AT THE CENTER BY THE FIRST DAY OF SCHOOL!!

* Health Records:

A child will not be allowed to enter school until a medical form has been fillled out (obtained at the doctor's office) and is on file at the center. HRS form DH 3040 and DH 680 are MANDATORY FOR ENTRANCE.

This form should state that the child has had:

1. A physical examination
2. All required immunization
3. TB skin test

* Birth Certificate and Social Security Card:

A photocopy of the child's birth certificate and social security card is required.
